The Urbana Police Division welcomes citizens' recognition of police employees performing in a quality manner. Citizen commendations will be forwarded to both the commended employee and to his or her personnel file.

CITIZEN COMPLAINT FORM
The Division and it's employees, are committed to maintaining integrity, discipline, and a relationship with the community of public trust and confidence. The Division has a "Professional Responsibility" process to accept and investigate allegations of employee misconduct. The Professional Responsibility process requires that all complaints be accepted, investigated, and then finished with a "conclusion of fact." If appropriate, employees will be disciplined for misconduct according to the Division's disciplinary procedures.

One day, you may become involved in an incident within the City of Urbana and the police division will request you to fill out a statement form. A written statement is important as it forms part of the evidence the police are collecting. The statement may be used to help decide whether a crime has been committed. Does the statement a witness provides give enough detail to warrant a criminal complaint?
A written statement protects everyone that is involved. It is your statement! It cannot be changed. When a statement is given orally, things can be added or omitted. In a written statement, nothing can be changed unless the person giving the statement is aware of it and agrees with those changes. If you have been requested to fill out a statement or have information in an investigation, you can print out the Urbana Police Division statement form.
